NOTE: In order to play soccer with the wallkill Valley Soccer Club, all three forms below must be printed, filled out completely, and returned to the Wallkill Valley Soccer Club along with a copy of the birth certificate and a small picture. Additionally, the Medical Release form must be Notarized before you submit it.
